Der King's Fragebogen zur Erfassung der Lebensqualität von Patientinnen mit Harninkontinenz (deutsche Version) (2024)

Zusammenfassung

Fragestellung: Wir präsentieren die psychometrische Testung der deutschen Version des King's Fragebogens und beschreiben den Auswertungsmodus. Patientinnen und Methodik: 145 Patientinnen, die wegen Stressinkontinenz behandelt wurden, füllten vor und nach der Behandlung den King's Fragebogen und den SF-36 aus. Zur Überprüfung der psychometrischen Kriterien wurden die Reliabilität, Validität und die Sensitivität ermittelt. Ergebnisse: Die Subskalen des King's Fragebogens zeigten eine gute interne Konsistenz (Cronbach's alpha 0,76 - 0,86). Für die deutsche Version wurde eine zusätzliche Subskala (Überaktive Blase) definiert, die ebenfalls gute psychometrische Kriterien aufwies. Die kriteriumsbezogene Validität zeigte statistisch signifikante Korrelationen zwischen dem Schweregrad der Inkontinenz und den King's-Subskalen: Inkontinenzbelastung (r = 0,22, p = 0,013), Persönliche Beziehungen (r = 0,24, p = 0,006) und Körperliche Einschränkungen (r = 0,22, p = 0,01). Bezüglich der Sensitivität zeigten sich statistisch signifikante Veränderungen vor und nach der Therapie in folgenden Subskalen: Inkontinenzbelastung, Körperliche Einschränkungen, Soziale Einschränkungen, Persönliche Beziehungen, Gefühlszustand und Schlaf/Energie. Schlussfolgerung: Die deutsche Version des King's Fragebogens weist gute psychometrische Eigenschaften auf und kann in klinischen Studien und in der klinischen Praxis zur Erfassung von Therapieeffekten bei Behandlung der Harninkontinenz eingesetzt werden.

Abstract

Purpose: We tested the psychometric properties of the German version of the King's Health questionnaire and describe the scoring procedure. Material and Methods: A total of 145 patients treated for stress incontinence completed the German-language King's Health Questionnaire and the SF-36 before and after treatment. A psychometric analysis of the instrument was performed, determining reliability, validity and responsiveness of the King's Health questionnaire. Results: The subscales of the King's Health Questionnaire showed a good internal consistency with Cronbach's alpha coefficients between 0.76 and 0.86. For the German version we defined a new subscale for overactive bladder. This subscale also showed a good internal consistency. The criterion validity measured on clinical parameters showed statistically significant correlations between severity of incontinence and several King's subscales: Incontinence Impact (r = 0.22, p = 0.013), Personal Relationships (r = 0.24, p = 0.006) and Physical Limitations (r = 0.22, p = 0.010). Regarding responsiveness statistically significant changes before and after therapy were found in the subscales: Incontinence Impact, Physical Limitations, Social Limitations, Personal Relationships, Emotional Problems, and Sleep and Energy Disturbances. Conclusion: The German version of the King's Health Questionnaire showed good psychometric properties and can be used in clinical trials and in clinical practice for the evaluation of treatments and their impact on urinary incontinence.
